The Hormones by Design Membership begins with a 3-month commitment designed to support meaningful progress. After your initial term, you may cancel at any time.
We meet with you at times that fit your busy life, so you can prioritize your health without disrupting your daily routine.
Virtual consultations with your providers means you don’t have to drive to a clinic, sit in a waiting room, or discuss private matters away from home.
We ensure that you’ll always speak with the same medical assistant, receptionist, and provider—your dedicated care team.
Membership applies only to injection services. Patients who choose not to receive injections will transition to a pay-as-you-go plan.

Hormones Direct is a convenient monthly membership from Hormones by Design that makes hormone therapy simple and predictable.
For $199 per month, you receive:
- Estradiol and/or testosterone injections shipped directly to your home
- A provider visit (virtual or in-person, when needed)
- Access to 25 commonly ordered labs at no extra charge
It’s designed to give you consistent care without surprise costs.
Hormones Direct may be right for you if:
- You live outside the area of one of our offices and don’t want to drive in
- You prefer a set monthly cost that’s easy to budget
- You value convenience and want medications delivered to your door

Hormone Injections Delivered to Your Home
- Estradiol and/or testosterone injections mailed directly to you
Provider Access
- Virtual visits (or in-person if desired)
Access to 25 Included Lab Tests Each Month (when medically necessary), including:
CBC, CMP, Cortisol AM, DHEA-S, Estradiol, Ferritin, Free T3, Free T4, FSH + LH, Hemoglobin A1c, hs-CRP, Insulin, Lipid Panel, Liver/Hepatic Function Panel, Progesterone, PSA (Free & Total), Sed Rate (ESR), Testosterone with SHBG, TPO & TG, TSH, Vitamin B12, Vitamin D.
If your provider recommends a lab that is not on this list, we’ll discuss any potential additional cost with you before ordering it.
Yes. Existing patients can switch to Hormones Direct at any time. Our team will simply have you sign a membership agreement.
Hormones Direct is currently designed only for patients using injections.
Yes. If you decide Hormones Direct isn’t right for you, you can switch back after an initial 3-month startup period.
If multiple injections have already been shipped to you (for example, a 3-month supply), you will need to complete payment through the end of that prescription period.
Your membership fee is billed automatically each month through our secure billing system. There’s no need to pay at the office.
Yes — your membership includes provider access.
If you transition to Nursing Only appointments (where injections are sent in 3-month supplies), you may not need to see your provider monthly unless:
- You request a visit
- You’re experiencing side effects
- Your provider recommends a check-in
Your monthly membership fee remains $199 regardless.
New patients will complete new patient paperwork plus a Hormones Direct Membership Agreement.
Existing patients only need to sign the Membership Agreement.
Hormone injections are filled through a specialty pharmacy and shipped to your home.
Other prescriptions (such as progesterone or thyroid medication) will be sent to your local pharmacy of choice.
If you choose to add pre-filled Semaglutide or Tirzepatide injections, the cost of that medication can be added to your monthly membership.
If you later decide to stop, simply call our office and we will remove it from your plan.
No problem. We provide:
- A step-by-step instructional PDF
- Guidance on safe injection technique
- Instructions on proper disposal of supplies
You’re also welcome to come into the office for an in-person demonstration or bring a spouse or support person to learn with you.
Yes. You may come into the office monthly for injections if you prefer.
Please note: Your membership fee remains $199 per month whether injections are shipped or administered in-office.
If you normally receive injections in the office but need them shipped while traveling, we can arrange that.
Just let us know ahead of time so we can ensure delivery before you leave.
If your provider recommends a lab outside of the included list, we’ll review it with you and discuss any additional cost before ordering. Our goal is always to avoid surprise charges whenever possible.
